Hello at work a coworker passed a napkin to me and it had a tiny red dot on it and I got worried it was blood. I have a cold sore and a few cuts on my hands am I at risk?

Casual contact with people such as you describe is not how we see HIV or STIs found in blood being passed. It is not considered a risk.
